Please arrive 10-15 minutes early for your appointment for registration and to complete a health questionnaire.

- Referral Letter (GP or other doctor)
- Medicare, DVA or Pension Card
- X Rays. MRI or CT Scans
- Private Hospital Insurance Details
- Copies of Pathology Reports
- List of current medication
- Your Medical & Family History
- Relevant Journal or Travel Plans
- Current Diet
- Employers Letter (if Workcover)
- Exercise Activities
Our reception staff will be happy to advise you of the consultation fee upon booking an appointment over the telephone. Fees for consultation along with other necessary forms will also be emailed to you for your convenience upon booking an appointment.
Surgical fees are billed directly to the Health Insurance Providers. There is sometimes an out-of pocket co-payment which can vary depending on the type and complexity of surgery or procedure. Patients will be advised, in writing, after the consultation, the amount of gap payment.
Fees for uninsured patients are advised by the accounts manager upon request.
This practice also caters for Work Cover patients and Department of Veteran Affairs (DVA) patients.
Payment on the day of consultation is much appreciated.

We understand that at times appointments need to be cancelled or rescheduled. If you need to cancel or reschedule your appointment please do so by phoning the practice on (07) 3345-6667 where one of our administration staff will be more than happy to assist you.
Please allow at least 1 day’s notice so that we can offer your spot to the patients on our consultation waiting list.
It is preferred that cancellations are not emailed to the practice.
This practice reserves the right to change its policy regarding cancellations without notice.
At the Sunnybank Obesity Centre we appreciate the importance of budgeting for procedures. Our practice manager Ms Sue Dobson (07) 3345-6667 will be more than happy to discuss costings of procedures with you so you can plan your finances.
If you do not speak English, we are happy for a friend or relative to call us on your behalf.
We will arrange for a qualified interpreter via telephone to help the provider communicate with the patient.
